READ THE POLICY, this is important
THIS FORM MUST BE FILLED OUT
FOR ALL TRAVEL, in
or out of state that involves per diem. This must be pre-approved by
the County Manager/Commission. There are no exceptions, and is a
county stipulation; beyond the Fire Administration. The form must be
signed by the traveler, signing for or by is not accepted; it must be the
traveler. The dates of travel and all supporting documentation is
required. Immediately return the form to the Fire Administration for
processing, no travel will be processed without the signature of the traveler.
The Supervisor is the McKinley County Fire Director even for entities that are
not McKinley County and other fiscal entities (Pueblo of Zuni, Tohatchi
EMS, Crownpoint NN EMS, Medstar Ambulance, Gallup Medflight and certain training
travel for Gallup FD Hazmat).
In state travel can be approved by the County Manager.
The lead time required usually takes about a week or so,
early submission is advised since the Manager may be out on travel at any
time.
All Travel requests for out of state travel
must be approved by the county commission. Due to this, it is advised
that all out of state travel be submitted at least a month ahead of time.
If submission of a request occurs in a period between commission meetings;
nothing can be done, and you will be denied travel reimbursement under Fire
Protection and EMS funding sources.
