THIS IS IMPETRATIVE DO NOT MISS OUT.
If you do not have the service credit you should,
you must during this year take advantage of the open credit allowance or you
will miss out in 2011. Only the individual Firefighter can do this, not
your chief and not the Administration. Of course we will help you.
VOLUNTEER FIREFIGHTERS - Information & forms for volunteer
firefighters

Volunteer Firefighter Enrollment Form (Posted
7/5/2009) - To be used by people enrolling in the volunteer firefighter
program.

Volunteer Firefighters Service Credit Qualification Record
(Posted 6/11/2009) - To be used by the
department chief to report service credit for the entire department. This form
must be received by PERA by March 31 of each year to ensure credit is given
for the individuals in the department.

Volunteer Firefighters Annual Reporting (Posted
6/11/2009) - To be used by the department chief to certify the number
of meeting, drills and responses the department had for the year.

Adjusted Qualification Record (Posted 7/5/2009)
- To be used by an individual Firefighter to adjust/correct service credit for
prior years which their department reported incorrectly or failed to report.
This form must be submitted with the "Adjusted Qualification Report".

Adjusted Qualification Report (Posted 7/5/2009)
- This form is to be signed by the member in front of a notary public
certifying the information on the "Adjusted Qualification Record" is true and
correct. This form must also be signed by the mayor or chair of the county
commission.
